INC.SPECIALTY PRODUCT LABOR WARRANTY AND SERVICE AGREEMENT

On the _____ day of ______________ 20____ (the “Date of Purchase”), the Customer named above (hereinafter
“CUSTOMER”), purchased the specialty product(s) described
on the attached invoice (hereinafter the “Specialty Product(s)), in new
condition, from MILLER MOBILITY PRODUCTS, INC. (hereinafter “MMP”). 
Except as provided
herein, the Specialty Product(s) sold by MMP in new condition are covered by
this Specialty Product Labor Warranty and Service Agreement (the “Agreement”).All Specialty Product(s) are covered by the
Standard One Year Labor Warranty, detailed below.Specialty Product(s) purchased by CUSTOMER
are only covered by the Extended Limited Labor Warranty, detailed below, if
CUSTOMER elects to purchase said Extended Limited Labor Warranty and pays the
fee(s) applicable to the Specialty Product(s) purchased by CUSTOMER.

Standard One Year Labor
Warranty

Subject to the terms and limitations set forth herein, for a period of one year from the Date of
Purchase, MMP shall provide labor, free of charge, to repair the Specialty Product(s).CUSTOMER shall be responsible for payment of
all trip charges associated with such repairs unless CUSTOMER elects to
purchase and pays the fees applicable to the Extend Limited Labor Warranty,
detailed below, in which event, said labor shall be performed by MMP at CUSTOMER’S
address, listed above, without any trip charge, provided, that CUSTOMER’S
address is within a forty (40) mile radius of MMP’s location in Oconomowoc,
Wisconsin (the “Coverage Area”).A trip
charge shall apply if the CUSTOMER’S address is outside of this Coverage Area.If applicable, trip charges shall be the
responsibility of CUSTOMER, shall be invoiced separately and payment shall be
due upon completion of the repairs/labor.

Extended Limited Labor
Warranty

(Only applies to new specialty product(s)
purchased after January 1, 2019)

After one year from
the Date of Purchase (hereinafter the “One Year Anniversary”), the Standard One
Year Labor Warranty described above shall no longer be effective and this Extended
Limited Labor Warranty, described herein, shall go into effect, but only if CUSTOMER elects to
purchase said Extended Limited Labor Warranty and pays the fee(s) applicable to
the Specialty Product(s) purchased by CUSTOMER.
(Please see MMP sales representative for the fee(s) applicable to CUSTOMER’S
purchase.)

If CUSTOMER does not
elect to purchase said Extended Limited Labor Warranty and/or fails to pay the
fee(s) applicable to the Specialty Product(s) purchased by CUSTOMER within 30
days fromthe date of product purchased,
then the Extended Limited Labor Warranty provisions detailed herein are NOT applicable to CUSTOMER’S purchase
of said Specialty Products.

Please Note:CUSTOMER may elect to purchase this Extended
Limited Labor Warranty at any time before the 30 day Anniversary, provided,
that CUSTOMER notifies MMP of said election and pays the fee(s)
applicable to the Specialty Product(s) purchased by CUSTOMER, before the 30
day Anniversary
, but CUSTOMER shall not be entitled to the refund of any
labor, trip or other charges paid by CUSTOMER prior to said election and/or
payment of applicable fees.

Under this Extended Limited
Labor Warranty, for a period of four years after the One Year Anniversary, but
subject to the terms and limitations set forth herein, MMP shall continue to provide
labor, free of charge, to repair the Specialty Product(s), at the CUSTOMER’S
address, without any trip charges, provided, that CUSTOMER’S address is within
the Coverage Area.A trip charge shall
apply if the CUSTOMER’S address is outside of the Coverage Area.If applicable, any such trip charges shall be
the responsibility of CUSTOMER, shall be invoiced separately and payment shall
be due upon completion of the repairs/labor.

In addition, under
this Extended Limited Labor Warranty, for a period of four years after the One
Year Anniversary, but subject to the terms and limitations set forth herein,
MMP shall also perform an annual Preventative Maintenance of the Specialty
Product(s) each year (i.e., one per year for a total of four), free of charge
and without any trip charge, provided, that CUSTOMER’S address is within the
Coverage Area.A trip charge shall apply
if the CUSTOMER’S address is outside of this Coverage Area.If applicable, any such trip charges shall be
the responsibility of CUSTOMER, shall be invoiced separately and payment shall
be due upon completion of the repairs/labor.

Each annual Preventative
Maintenance must be scheduled by CUSTOMER and occur within one year of the previous
anniversary date of the Date of Purchase.
Annual Preventative Maintenance will only be performed during regular
business hours, Monday through Friday (excluding holidays).CUSTOMER is entitled to only one annual
Preventative Maintenance visit by MMP each year during the four year extended
labor warranty period.

IMPORTANT!!IF CUSTOMER ELECTS TO PURCHASE THE EXTENDED
LIMITED LABOR WARRANTY AND PAYS THE APPLICABLE FEE(S), AS DETAILED HEREIN,
CUSTOMER MUST HAVE THE PREVENTATIVE MAINTENANCE PERFORMED BY MMP ON AN ANNUAL
BASIS.FAILURE TO HAVE THE PREVENTATIVE
MAINTENANCE PERFORMED ON AN ANNUAL BASIS WILL VOID THE EXTENDED LIMITED LABOR
WARRANTY.IN SUCH EVENT, CUSTOMER WILL
NOT BE ENTITLED TO ANY REFUND OF FEES PAID.

By marking a line
below, the CUSTOMER elects:

elects to purchase the Extended Limited Labor Warranty and agrees to pay the
fees applicable to the Specialty Product purchased by CUSTOMER.

_____CUSTOMER
declines to purchase the Extended Limited Labor Warranty but understands that
he/she may still elect topurchase this Extended
Limited Labor Warranty at any time before the 30 Day Anniversary, provided,
that CUSTOMER notifies MMP of said election and pays the fee(s)
applicable to the Specialty Product(s) purchased by CUSTOMER, before the One
Year Anniversary

Terms and Conditions
Applicable to Both Standard One Year and Extended Limited Labor Warranties

Neither the Standard
One Year Labor Warranty nor the Extended Limited Labor Warranty, described
above, includes the costs of parts needed to perform the repairs/labor.The cost of all parts shall be the
responsibility of CUSTOMER, shall be invoiced separately and shall be due upon
completion of the repairs/labor.

This Agreement is
not a warranty of any parts that may be used by MMP in the repairs/labor it
performs. The parts manufacturer is
solely responsible for any warranties associated with the parts it
manufactures. If parts are no longer available from the manufacturer, MMP will
offer comparable new or used replacement parts at CUSTOMER’S expense.

THE LABOR WARRANTIES PROVIDED IN THIS
AGREEMENT SHALL NOT APPLY TO REPAIRS REQUIRED DUE TO PRODUCT NEGLECT, MISUSE,
ABUSE, MODIFICATIONS, ACCIDENTS,IMPROPER STORAGE ENVIRONMENT, UNUSUAL
PHYSICAL OR ELECTRICAL STRESS OR INTERFERENCE, LIGHTNING, STATIC ELECTRICITY, FIRE
OR ACTS OF GOD.THESE LABOR WARRANTIES
APPLY ONLY TO REPAIRS REQUIRED AS THE RESULT OF NORMAL WEAR AND TEAR, OR THOSE
REQUIRED DUE TO DEFECTS IN MATERIALS OR WORKMANSHIP.REPAIRS PERFORMED PURSUANT TO THESE LABOR
WARRANTIES SHALL BE REPAIRS RECOMMENDED BY THE SPECIALTY PRODUCT(S)
MANUFACTURER AND SHALL BE PERFORMED ACCORDING TO MANUFACTURER SPECIFICATIONS.

In the event that,
during a service trip or Preventative Maintenance, MMP provides additional
services not covered by this Agreement, the labor, service trip charge and/or
costs of parts for such additional services shall be separately invoiced.All such fees, labor charges, service trip
charges, cost of parts and emergency service fees shall be due upon completion
of the repairs/labor.CUSTOMER’S
requests for emergency repairs shall result in additional labor and/or trip
charges.

CUSTOMER
acknowledges and understands that MMP is in the business of selling, renting
and servicing mobility products including the Specialty Product(s), and that MMP
is not in the business of representing or warranting the condition of such products;
whether such products meet applicable municipal, state, local or governmental
codes; whether the mechanical systems of such products are in good working
condition; or whether there are other defects in such products.

CUSTOMER hereby
indemnifies and holds MMP harmless for any and all damages which may occur
while performing the repairs/labor provided for under the terms and conditions
of this Agreement.

This Agreement and
the rights and duties hereunder may not be assigned, transferred or delegated
by either party without the prior written consent of the non-assigning
party.Unless agreed to by MMP in writing, the
Warranties describe herein are for the CUSTOMER only.

This Agreement shall
be construed under the laws of the State of Wisconsin and the Circuit Court for
Waukesha County, Wisconsin shall have jurisdiction to hear any and all disputes
under the terms and condition of this Agreement.

This Agreement
represents the complete understanding of the parties with respect to the
subject matter set forth herein, and may only be amended by subsequent writing
executed by both parties. The invalidity or unenforceability of any provision hereof
shall in no way effect the validity or enforceability of any other provision.

This Agreement constitutes the entire Agreement between the parties, and
all understandings and agreements heretofore made between them concerning this
matter are merged into this Agreement.
